CONSENT TO JOIN SETTLEMENT, RELEASE, AND CLAIM FORM

This signed form must be returned to the Settlement Administrator so that it is postmarked or submitted online by October 19, 2026.

I hereby consent to join and opt-in as a claimant in the Sovos Overtime Settlement and to be bound by any adjudication of this action by the Arbitrator.  I hereby designate the law firm of Outten & Golden LLP to represent me in this action.  I understand by returning this Claim Form, I will fully and completely waive, release, and discharge the Releasees (as defined in the Settlement Agreement) from the Released Collective Claims (as defined in the Settlement Agreement). The release covers claims pled in the arbitration demand or that could have been pled based on the facts alleged in the arbitration demand, and that accrued during my employment at Sovos as a Compliance Services Employee from April 4, 2022 through February 24, 2026, including, without limitations, federal, state, and local claims for unpaid wages, overtime wages, improper wage statement, missed breaks, waiting time, and related claims for penalties, interest, liquidated damages, attorneys’ fees, costs, and expenses. I understand that, in exchange for the release, I will be entitled to a settlement payment as calculated under the Settlement Agreement.
